Once your registration and payment is recieved you will recieve a confirmation email.
Policies
Health History and Examination Form:
The health of each student is an important concern. All students are required to have a medical examination at least two years prior to their participation in Project Peak. The four-page student Health History and Examination form must be completed on all sides and signed by an examining physician (and the parent/guardian if student is under 18 years of age). Note: You can complete the first three pages of the four-page form. Your physician only has to complete the last page. If you have had a physical within the last two years, just complete the information and have your doctor sign the document based on that exam. Bring the completed form with you to check-in on August 17th. Any student arriving without a completed medical form will not be permitted to participate in Project Peak. In case of medical emergency, the Project Peak faculty member and/or the Project Peak Coordinator will select a physician who will make decisions regarding the treatment, hospitalization, injections, anesthesia, or surgery, which he/she deems necessary. If such an emergency occurs, every effort will be made to immediately contact parents/guardians.
Note: This form requires medical information that will also be required by Health Services as part of your enrollment process; complete both forms and make sure that The Freshman Center receives the form in this packet, as the information will not be given or shared with any department outside of a The Freshman Center.
Medications
All medications must be listed on the Health and Medical form. All medications, even over the counter medications, must be in their original bottles. The trip leaders are not permitted to administer any drugs.
Liability Forms
Each student must sign each of the four liability release forms provided (links above). Parents/Guardians must sign their forms if the student is under 18 years of age. Bring the signed forms with you to check-in on August 17th. Any student arriving without these signed forms will not be permitted to participate in those specific activities.
Conduct
The Project Peak staff reserves the right to send home any student whose behavior is considered by the trip leaders or Program Coordinator to be detrimental to the best interests of the program, for example, the use of alcoholic beverages, illegal drugs, refusal to cooperate and/or follow rules and regulations, refusal to accept his or her share of responsibility, or endangering the health or safety of any other person on the trip. No refund will be given for a disciplinary dismissal.
Cancellation
Registration fees are refundable only if that spot can be filled with another student, less an administrative fee of $50. No deductions are permitted for late arrivals or early departures. However, any student forced to leave due to injury or illness, will be refunded a portion of their registration fee.
